Overview:

The City of Stonnington provides specific Home Care for people caring for people with a physical, intellectual or neurological disability or sensory impairment. Specifically trained staff will provide personal care and general assistance to the person with a disability in their own home during weekdays, evenings or weekends to enable the carer to have a break.

Stonnington Community Care In Home Support services are available to eligible City of Stonnington residents. Services are targeted towards frail older people, people with a disability, and the carers of both. Eligibility and access to services is subject to a number of factors, and does not necessarily ensure entitlement to receive services. Services are allocated to eligible residents according to assessed and prioritised need.

Residents should telephone Stonnington Community Care directly, or can be referred by someone else (with consent) such as a family member or friend.
